Transitional & Rehabilitative Services
At Symphony Maple Crest, we have improved and enhanced transitional and rehabilitative services wholly for the benefit of our guests. Each guest is assigned a “Transitions Coach.” The Transitions Coach manages the medical and logistical needs required to ensure a smooth transition from hospital to home while promoting function, safety, and independence in as short a stay as possible.

The focus of this program is on achieving the highest level of functional and medical recovery through individually designed rehabilitation care plans.

TRANSITIONAL REHABILITATION
Transitional rehabilitation guests are monitored by a physiatrist through a continuum of care. Guests pursue an individualized care plan, designed by a Physician, Rehabilitation Team, and Discharge Coordinator.
As guests progress in the therapy program, their care plan is modified and new goals are added moving them toward a higher level of function and ultimately independence. Prior to discharge, our staff works with each guest to ensure that their activities of daily living can be met and assess the home environment, if needed.

Physical Therapy

Our cardiac rehabilitation program promotes optimum recovery and health for those recovering from heart disease and/or cardiac-related surgery. The goal of the program is to limit the physical effects of cardiac illness and improve cardiovascular fitness and overall health. Each of our guests receives a program designed specifically for them, supervised and implemented by a team of cardiac specialists, including physicians, nurses, physical and occupational therapists, and consulting nutritional and psychological professionals.
Symphony Maple Crest has developed a pulmonary program designed to meet the needs of patients with chronic and acute respiratory illnesses. Our post-hospital Pulmonary Rehabilitation Program is tailored to meet each individual’s needs and manages such illnesses as:

The Pulmonary Rehabilitation Program is designed to control symptoms, improve quality of life and return patients to their optimal level of functioning. The highly skilled inter-disciplinary team will work together to create a personalized plan that addresses patients’ needs and monitors their day-to-day progress. The program maximizes patients’ independent functioning and prepares them for a safe return home.
Symphony Maple Crest offers a comprehensive orthopedic rehabilitation program that provides our guests all the therapeutic, clinical and technical resources needed to address a wide range of orthopedic conditions, including:
